What this site is

The Care Binder publishes general reference material about the paperwork of long-term care for older adults in the United States: nursing facility and assisted living bills, residency agreements and admission packets, Medicare and Medicaid coverage notices and appeals, powers of attorney and other authority documents, public inspection records, home care agency contracts, and benefit programmes such as HCBS waivers, PACE, and VA Aid and Attendance. It explains what those documents say, which regulation or contract clause each statement comes from, and what deadlines the published rules set.

It is a reference, not a service. No page here reviews your contract, reads your notice, assesses your parent's needs, checks anyone's eligibility, prepares or files anything on your behalf, or communicates with a facility, agency, insurer, or government office for you. Reading this site does not put anyone to work on your family's situation.

This site is not a placement or referral service and is not affiliated with any facility, agency, insurer, or eldercare business. It accepts no referral fees, placement commissions, or sponsored listings, and it does not sell or pass on reader enquiries. Nothing here is an endorsement of any named facility, community, agency, or provider; where one is named, it is because a public record or document is being described.

Two limits are worth stating plainly, because they are the two things this subject invites:

  • No page here makes a medical or care decision. Nothing on this site says whether a person needs care, what level of care they need, whether they should move into or leave a facility, or what treatment is appropriate. Those judgements belong with the person, their family, and their clinicians.
  • No page here tells you whether to sign, or advises you on your legal rights in your own case. Where a contract clause or a regulation is described, it is described as text — what the wording says and where it is written down — so that you can read your own document and discuss it with a lawyer if it matters. Whether a clause is enforceable against you, and what it means for your family, is a question for a qualified professional in your state.

Long-term care is governed by your own signed agreements, by the rules of the specific programme paying the bill, and by state law — and assisted living licensing, Medicaid eligibility, guardianship, advance directive formalities, and waiver waiting lists genuinely differ from one state to the next. General material cannot resolve that for you. For anything binding, consult an elder law attorney, your state Medicaid agency, your State Health Insurance Assistance Program, your long-term care ombudsman, Medicare, or the Department of Veterans Affairs.

Accuracy and currency

Guides are researched against official sources and dated on publication. Even so:

  • Medicare coinsurance amounts, Medicaid income and asset limits, VA pension rates, and facility rates are reset at least annually; appeal deadlines, notice forms, and programme rules change by regulation; and a state can rewrite its assisted living rules or its waiver waiting list policy at any time.
  • A guide reflects the situation at the date shown on it, not necessarily today.
  • Errors happen.
